Our services are available to residents of continuing care communities, including independent living, assisted living, and memory care settings. In many cases, medical services provided in these communities are covered by insurance, allowing residents to receive high-quality primary care with minimal out-of-pocket costs.
We work closely with patients, families, and community staff to ensure care is well-coordinated, accessible, and tailored to each individual’s health needs and living environment.
In-Person Visit & Communication Policy for Residents
In-person visits are provided to residents as medically necessary and are scheduled based on changes in the resident’s clinical condition and overall health status. The frequency and type of visit are determined by individual medical needs and provider assessment.
To support timely and coordinated care, our team is available through multiple communication channels, including telehealth visits, patient portal messaging, phone, text, fax, and email. We maintain bidirectional communication to ensure ongoing collaboration with residents, families, and care teams.
Aging-in-Place Home Care Program Model
For patients who choose to receive care in the comfort of their own home, our aging-in-place care model includes a one-time initiation fee and a monthly access fee. These fees reflect the additional time, travel, and coordination required to deliver personalized, high-touch medical care in a private home setting.
Medical services continue to be billed to and reimbursed by insurance when applicable. The monthly access fee supports enhanced availability, extended visit time, and the convenience of comprehensive in-home care tailored to each patient’s individual needs.
